Clutch adjustment

I have T56 transmission with a hydraulic slave cylinder. I can't seem to get the thing adjusted properly! What is the proper way of adjusting a clutch?
 
It is external sternal slave cylinder. To me it sounds like the throwout bearing isn't fully disengaging. It started this weekend when I took the car down to the Salem roadster show. It's starting to make almost a rattling sound in the area of the bell housing. Nothing too dramatic but it seemed to get progressively worse.

I know I miss suggested my last clutch. There's only in the car for approximately 2000 miles and it was pretty much worn-out. I adjusted this clutch by loosening up a little bit and until I could feel play in the arm. I have had one person tell me I should adjusted back out until it just starts grinding when you put it in gear then tighten a little from there. This is my next step unless somebody else has a better plan.
 
I have the modern hydraulic for my T5 and I have to keep it pre-loaded. It kinda goes against everything I believed about throw out bearings going south due to constant pressure but thats what I had to do. In fact I removed the pivot post in my bell housing and put a couple of washers behind it to move the pivot point a bit, then I adjusted the rod on the slave to pre-load the thing a fair bit.
 
It makes me wonder if I have a bit too much preload. I think I will back it off and see how it goes. I need to get the thing adjusted in the next week or it will sit there for a month or two. I have k we surgery on my left knee on the 21st. :(
