I think I solved my problem. After looking at images of slip yokes on the net, I noticed that they all have a plug at the rear of the yoke closest to the u-joint. Well, my slip yoke is missing the rear plug! Oil has been leaking out the slip yoke between the output shaft splines and slip yoke spline.
I spoke to my drive shaft builder, and he said it could be caused by one of two things.
1) The drive shaft is too long, and the output shaft popped the plug out of the slip yoke.
or
2) During installation I pushed the drive shaft too far forward, which punched out the plug.
I'm going to reinstall the driveshaft to check how much clearance the slip yoke has to travel, but from what I described to the drive shaft builder, it sounds like the shaft is the right length.
Therefore, I think it's #2, because I remember struggling to align my drive shaft & slip yoke with the trans output shaft. When slip yoke finally aligned with the trans output shaft, it slid in with a bang. That aggressive installation probably knocked out the rear plug of the slip yoke.
If the drive shaft is NOT too long (hopefully, it isn't), I'll need to find a washer or freeze plug to weld onto the rear of the slip yoke to stop the trans fluid from leaking out of the back of the slip yoke.
